  • This Safety Alert is only applicable to the harnesses specified, for retailers or consumers based in New Zealand.

    BEFORE STARTING THIS FORM, PLEASE DO THE FOLLOWING:

    Have your harness serial number available. We cannot process this request without a Valid Serial Number [##@##########]. We can also NOT process duplicate serial numbers, so if a duplicate serial number is received, that harness will be deleted from our list.

    As each harness has a unique serial number, you will need to complete an entry for each eligible harness you have. If you have multiple of the same model, please select [+ Add Row] to list another harness. Serial Numbers should be lower than 23J 9999999 999.
  • PLEASE ONLY ENTER ONE HARNESS/SERIAL NUMBER PER ROW. DO NOT PASTE BULK SERIAL NUMBERS INTO THE SERIAL NUMBER BOX AS THE FORM WILL BE REJECTED.

    File format must be a CSV format only,
    You must have 3 columns only, with the headings:
        MODEL NAME,     MODEL REF #,    SERIAL NUMBER
    All 3 columns data must be completed,
    You can include any of the affected models in the one file.
