Project Overview
The project’s primary aims are to keep older people out of hospital and reduce the frequent use of urgent and emergency care (UEC) services during the winter period by providing targeted interventions and support toolder people in Preston who are at risk of injury due to falls. The initiative will focus on supporting frail older adults, particularly those with existing long term health conditions, and at high risk of falling. The project should focus on people living within their own homes, including those who are supported by both paid and unpaid carers, as well as considering individuals residing in a care home setting.

The project will deliver proactive outreach, the development of preventive interventions, and work in close partnership with local health and social care organisation’s, to avoid duplication add value, support individuals to access existing services where available, and address unmet need where gaps exist.

Question 1 – Understanding of the Community and Target Cohort

Based on the specification, please describe your understanding of the needs of older residents within the PR1 and PR2 post code areas, particularly those who are at risk of injury due to falls, and wider issues associated with deprivation and multiple disadvantage.

Please explain how your organisation’s experience and local knowledge will help you engage and support the target cohort identified within the specification.

Question 2 – Delivery Model and Approach

Please describe how you will deliver the project and meet the requirements of the specification.

Your response should include:

• How you will identify, risk assess and proactively engage older people living in PR1 and PR2 who are living with frailty, long-term conditions and/or are at increased risk of falls.
• Your approach to proactive outreach across people’s own homes, care homes and community settings, including how you will engage unpaid carers, families and those who may be socially isolated.
• How you will undertake holistic, person-centred assessments that identify both falls risks and wider health and wellbeing factors that may contribute to avoidable hospital attendance.
• How you will develop and deliver personalised falls prevention plans, including home safety, strength and balance interventions, assistive technology, welfare support and wider preventative measures.
• How you will support people to remain independent, improve confidence and safely manage within their own homes during the winter period.
• How you will connect individuals to existing community, voluntary, health and social care services, ensuring that unmet need is identified and duplication of support is avoided.
• How you will ensure your approach addresses health inequalities and effectively reaches those most at risk.

Question 3 – Partnership Working, Integrated Care and System Collaboration

A key requirement of this project is that delivery is fully integrated with local health, social care and community services to provide coordinated support and reduce avoidable urgent and emergency care demand.

Please explain how your organisation will work collaboratively as part of this integrated model rather than as a stand-alone service.

Your response should include:

• How you will work alongside GPs, Primary Care Networks, the Transfer of Care Hub (ToCH), community health services, social care, care homes and voluntary sector partners.
• Your approach to participating in multidisciplinary team (MDT) meetings, joint case management and coordinated care planning.
• How you will establish effective referral pathways into services such as physiotherapy, occupational therapy, falls services, medication reviews and other preventative support.
• How you will work collaboratively with care home staff, unpaid carers and families to improve falls prevention and ongoing risk management.
• Examples of previous partnership working that demonstrate successful integrated delivery, improved resident outcomes and reduced pressure on health and care services.
• How you will ensure information sharing, communication and governance arrangements support safe, timely and coordinated delivery.

Question 4 – Outcomes, Impact, Learning and Continuous Improvement

Please explain how you will monitor, evidence and demonstrate the impact of the project throughout the six-month delivery period and contribute to the End of Project Summary.

Your response should include:

• How you will collect, monitor and report activity, outputs and outcomes throughout the project.
• How you will evidence improvements in falls risk, independence, confidence, wellbeing and ability to remain safely at home.
• How you will measure the effectiveness of personalised interventions, including home safety improvements, education, technology and preventative support.
• How you will monitor referrals, service uptake and successful connections into wider community and statutory services.
• How you will evidence the project’s contribution towards reducing falls, avoidable A&E attendances, hospital admissions and wider urgent and emergency care demand.
• How you will gather service user, family, carer and care home feedback to inform continuous improvement.
• How you will produce high-quality case studies and contribute to the End of Project Summary, including findings, learning, impact and recommendations that will inform future service development.
