1. Integrated Model for Admission Avoidance - Describe your proposed model for delivering community-based care coordination across the Lancaster, Morecambe, and Carnforth ICC footprint. How will your approach directly contribute to preventing avoidable hospital admissions for all-age patients known to the Royal Lancaster Infirmary and ICC Teams? Include how your model will align with key system partners (e.g. ICCs, PCNs, Transfer of Care Hubs, Urgent Community Response) and establish effective referral pathways that target those at highest risk of readmission or escalation to urgent and emergency care (UEC)?
