1. I have decided to fill the form after reading all the details carefully.
2.I am ready to pay the main form fee of 315INR.
3.If the details filled in the form by me turn out to be wrong then my form should be rejected.
4.If any wrong image of any document is uploaded by me in the form then action should be taken against me under IT Act.
5.I want to fill the form by accepting all the terms and conditions given in the form.

2. मैं मुख्य फॉर्म शुल्क 315 रुपये का भुगतान करने के लिए तैयार हूँ।
3. यदि मेरे द्वारा फॉर्म में भरी गई जानकारी गलत पाई जाती है तो मेरा फॉर्म अस्वीकार कर दिया जाए।
4.यदि मेरे द्वारा फॉर्म में किसी भी दस्तावेज की गलत छवि अपलोड की जाती है तो आईटी एक्ट के तहत मेरे खिलाफ कार्रवाई की जानी चाहिए।
5.मैं फॉर्म में दी गई सभी शर्तों को स्वीकार करके फॉर्म भरना चाहता/चाहती हूं।
