I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing and wearing a mask inside this business.
I further acknowledge that Substance Salon has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Substance Salon can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, salon staff, and other salon clients and their families.
I voluntarily seek services provided by Substance Salon and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the US or UK in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Substance Salon harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from Substance Salon. I understand that this release discharges Substance Salon from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Substance Salon. This liability waiver and release extends to the salon together with all owners, partners, and employees.
Substance Salon is following these enhanced procedures to prevent the spread of COVID-19:
• Contactless payment using the credit card on file, Apple Pay, or similar contactless payment methods. Cash will not be accepted as a form of payment during Phase l of operations. Tips will be accepted in Venmo or cash in an envelope for safety.
•. All surfaces will be wiped thoroughly with hospital grade disinfectant before and after each client according to the manufacturer's directions
Our Team and clients are required to wear a face mask, upon entry into Substance Salon, in accordance with NJ safety guidelines
•All Substance Salon employees and clients will have their temperature checked upon entering the building.
Substance Salon will only service clients with appointments. All others will have to wait outside.
• Each Substance Salon employee and client is required to wash/sanitize their hands upon entering the building.
•Each service provider will wash their hands and wear a face mask for each client.
Clients are allowed one personal item with them in the building.
We will not serve beverages, food or reading materials.
•Please be advised our service menu has changed and prices have been adjusted. Service providers may be tipped via Venmo or cash in an envelope.
• All tools will be sterilized before and after each client.
•All product bottles used during your appointment will be placed in a separate container for disinfection, and disposable items will be used when possible.
Should you forget your mask, Substance Salon will have masks available for purchase.
In an effort to minimize unnecessary exposure to surfaces for the time being, our staff will not be able to touch coins, dollars or credit cards for the meters. Clients will be responsible to monitor their meters.
• Please be advised we currently do not support refunds of any type. All purchases and services are final. If you have any concerns about your service please present your concern to our front desk during the check out process. If you are unhappy with your service and have notified us before leaving the salon we will schedule you for a follow up appointment with the service provider to make adjustments to your hair. By signing this document you understand and acknowledge our refund policy.
I agree to each statement above and release Substance Salon from any and all liability for the unintentional exposure or harm due to COVID-19
