This form must be completed for an in-house activity that will have fully registered officials on deck but that times will not be recognized. Such as mock meets within club training hours.

The activity leader must complete this form.

The head coach must acknowledge that the club is aware of this activity taking place.

The Club president may be contacted for confirmation.

The club must wait until receiving approval before commencing the activity.

This does not apply for other club activities such as banquets, swim-a-thon, etc. Only activities in which Officials will be acting as officials (i.e. referee, starter, timers).
