This form can be completed for swimmers that break Provincial and/or National Records including Para & Masters records.

For non-Ontario swimmers you can complete the Canadian Record Application form found at https://www.tfaforms.com/4628573 , however the Ontario record Application form would be sufficient as all information will be sent to Swimming Canada via the proper channels

If incorrect information is entered, the person submitting the application or the meet manager will be contacted. The record application form and required paperwork must be submitted to Swim Ontario within 7 days of the performance.
