This form is for scheduling a tattoo appointment with Kal Morrison in Los Angeles, Ca. After completing and submitting this form you will be emailed studio information and a confimation of the appointment.

A $100 deposit will be required to secure appointment spot. The deposit amount is deducted from the final price of the tattoo the day of your appointment. Deposits are non-refundable and non-transferable and secure a specific appointment time. By submitting this form and paying a deposit, you are agreeing to these terms. No exceptions.
