  • Provide your responses to the item below as appropriate based on the following guidance:

    Letter of Intent: (Evaluating free from grammatical errors, appropriate sentence structure, and how well they represent themselves)

    • 4 (Excellent) - Letter lists future goals, pharmacy practice philosophy, how the applicant plans to use the scholarship and what it means to the applicant if he/she receives it. Applicant shows leadership interest or need (for need-based scholarship), and desire to improve the profession of pharmacy.
    • 3 (Very Good) - It does not address all of the following: future goals, pharmacy practice philosophy, how the applicant plans to use the scholarship and what it means to the applicant if he/she receives it.  
    • 2 (Good) - Generic letter that describes direction in congruence with the goals of the scholarship and desire to improve the practice of pharmacy and be involved in organizations.
    • 1 (Average) - Letter does not add to the information about the applicant. Letter detracts from the applicant’s submission, misspellings, misgivings about the profession
    • 0 (Poor) - No Letter.
  • Provide your responses to the item below as appropriate based on the following guidance:

    Strength of Reference's Recommendations: (Evaluating leadership, clinical knowledge, personality, motivation, time management)

    • 4 (Excellent) - References give applicant their highest recommendation, point out strengths and interests in congruence with the scholarship, and encourage us to consider this individual our highest ranking candidate.
    • 3 (Very Good) - References give applicant a high recommendation, explain strengths, and encourage us to consider this applicant for scholarship.
    • 2 (Good) - References give applicant an average recommendation.
    • 1 (Average) - References give examples of multiple weaknesses of the candidate and encourages us to not to consider the applicant due to deficiencies of professionalism were identified or issues with peer/”supervisor” interaction.
    • 0 (Poor) - Did not recommend applicant.
  • Provide your responses to the item below as appropriate based on the following guidance:

    Leadership and Involvement in Extracurricular Activities:

    • 4 (Excellent) - Was president or vice president (ambassador/class representative) of a student organization in the college of pharmacy or within the university, held a student representative position at a state or national level, or led a community service initiative. Was recognized for their involvement in these groups, and used their leadership to benefit the profession. Strong participation in TSHP.
    • 3 (Very Good) - Was an officer of a student organization in the college of pharmacy or within the university or held a student representative position at a state or national level.
    • 2 (Good) - Participated in community service projects or student organizations (more than one), but never took a leadership role.
    • 1 (Average) - Participated in one or less community service projects or student organizations, but never took a leadership role.
    • 0 (Poor) - No leadership or community service.
  • Provide your responses to the item below as appropriate based on the following guidance:

    GPA:

    • 4 (Excellent) - Applicant has 3.80-4.00 in their pharmacy curriculum
    • 3 (Very Good) - Applicant has 3.60-3.79 in their pharmacy curriculum
    • 2 (Good) - Applicant has 3.30-3.59 in their pharmacy curriculum
    • 1 (Average) - Applicant has 3.00-3.29 in their pharmacy curriculum
    • 0 (Poor) - Applicant has <3.0 in their pharmacy curriculum

    Previous Work Experience:

    • 4 (Excellent) - Worked for >2 years in a hospital setting
    • 3 (Very Good) - Worked for equivalent to one summer to 2 years in a hospital setting or did a pharmacy internship.
    • 2 (Good) - Worked for less than one summer in a hospital setting and the rest of the experience is community
    • 1 (Average) - Worked all in community pharmacy.
    • 0 (Poor) - No work experience.
