INFORMATION SHEET
Purpose/Description: You are invited to participate in the TSU SHAPE Initiative Healthy Connections Hub. This is a campus and community-based initiative that engages adults 18 and older about their sexual health and wellness perspectives in reference to specific prompts. Training videos reflect three education skits about sexual health and wellness along with a pre and post assessment associated with each skit.
You are being asked to electronically complete the preassessment, view the skit (3 minutes or less), and complete the post assessment for each skit.
Risk: The probability and magnitude of harm or discomfort anticipated in the research are not greater in and of themselves than those ordinarily encountered in daily life.
Benefits: You are not expected to receive any direct benefit as a result of the research; however, you will be provided with information on how to access the TSU SHAPE Initiative for free sexual health and wellness services.
Voluntary Participation: Your participation is voluntary, and you may choose to not participate at all, or cease participation prior to the completion. You will not be penalized in any manner for not participating.
Compensation/Incentives: You will receive no compensation for your participation; however, as an token of appreciation for your time the first 175 viewers viewing all three scripts and completing each pre and post assessment will be eligible to receive a one-time $10 Amazon gift card by email within 5-7 business days.
Confidentiality: Your identity will not be attached to your responses, and we will have no way of linking you with your pre or post assessments (you will remain anonymous).
Records (Electronic): Electronic data records will be password protected behind the TSU university firewall. Only the study investigator and TSI program staff will have password access.
