    Doors will open at 9:00 AM for check-in. The program starts at 10:00 AM

    Attendance is limited to 350 on a first-come, first-served basis

    Registration is open March 1 – June 25, 2026

    One form is required per person

  • How to Register

    Attendance at Assembly Day is free for TVQA members but you must be registered.  There is a maximum available of 350 attendees.

    You may also select the optional box lunch for $13.50.  Lunches come with chips, dill pickle spear and a mini moon-pie.  Mustard & Mayo packets available.  Water will be available at no cost.

    • The Flattie-Flat panini bread, honey roasted turkey, provolone cheese, spinach, tomato
    • The Ham & Cheese-Cuban bread, smoked ham, American cheese, green leaf lettuce, spinach, tomato
    • Chicken Salad-house made chicken salad on ciabatta bread
    • The Big Herb-Cuban bread, Swiss cheese, green leaf lettuce, spinach, tomato, onion, bell pepper, cucumber & pickles

    Additional fields will appear and you can make your selections.

    One form per person.

    Lunch Payment

    If selected, TVQA is pleased to offer 2 options for payment for lunch

    • Online via PayPal - online payment fee applies
    • Mail/Check - mail a personal check or have your bank send a check to TVQA (address in confirmation email).

    Once payment is received, you will receive confirmation.

  • Submission

    To submit your Assembly Day Registration with no lunch, click the Submit Registration button below.

  • Payment

  • Click Pay by CHECK to submit your Assembly Day Registration and Lunch order, pending payment.

    Your registration is not submitted until "Pay by CHECK" has been clicked. 

    You will receive an email acknowledgement of your order.  Once payment has been received, you will receive an email confirmation.

  • If everything is correct, click PayPal Checkout or Venmo Checkout to submit your online payment and Assembly Day Registration.

    Once inside the PayPal portal, you can select to pay using your PayPal account OR by credit or debit card. 

    If "create PayPal account" is checked and you do NOT want to create a PayPal account, uncheck that box.  IF YOU SELECT PAYPAL CREDIT CHECKOUT, IT WILL SIGN YOU UP FOR A PAYPAL CREDIT CARD!!!

    PayPal will return to the form...give it a few seconds to complete the transaction. You will see a Thank You page when completed.

    You will receive an email confirmation from noreply@jotform.com.  Check your spam folder if you do not see this in your email.

    If there is an issue with your guild membership status, you will be notified.
