I represent that I am the legal owner or authorized by the owner of the dog(s) receiving services.

I understand that as a requirement, when receiving services from Furr'd by T. Reid, LLC my dog(s) must wear a safety collar/harness and leash.

I represent that my dog(s) is in good health and is currently up to date on all required vaccinations including Rabies, Bordetella (Kennel Cough), and DPP (Distemper, Parvovirus & Parainfluenza I represent that my dog(s) is free of fleas, ticks, and lice and has not been ill within the last 30 days.

I understand that while my dog(s) is fully vaccinated, vaccines are not guaranteed and there is a small risk that my dog(s) may contact a contagious disease or illness. I agree that should this occur, I am responsible for my own pet's care, medical attention, and costs. I agree to send the update vaccination record to Furr'd by T. Reid, LLC at info@getfurrd.com.

I release Furr'd by T. Reid, LLC, its staff, owners, and any representatives from any and all liability which I or my dog(s) may suffer including but not limited to injury, sickness, damage, or death resulting from participation in boarding services.

 
I understand that although all dogs are fully supervised, incidents of injuries may occur from playing with other dogs, which include but are not limited to bites, scrapes, scratches, and sprains.

I represent that my dog(s) is social and has not harmed or shown threatening behaviors towards any person or other dog. I understand that Furr'd by T. Reid, LLC reserves the right to abruptly end services for my dog if undesired and dangerous behaviors are displayed.

 
While my dog(s) is/are in the care and custody of Furr'd by T. Reid, LLC, in the event of an emergency that requires medical attention, I hereby authorize its agents and/or representatives to seek immediate veterinary care. I agree that I am solely responsible for any medical expenses acquired for my dog(s).

In the event that I or my authorized contact cannot pick up my dog(s) at the agreed pick-up time, I authorize Furr'd by T. Reid, LLC to provide additional overnight boarding services at my expense.

I understand that if I pick up my dog(s) after the scheduled pick-up time, I will incur additional fees.

I understand that there is an additional $10 holiday fee when Furr'd provides a service on a holiday.

 
I understand and represent that the invoice for boarding services will be paid in full 24 hours prior to drop off time for the service. If not, I understand that Furr'd by T. Reid, LLC. will have the right to cancel scheduled services.

Furr'd by T. Reid, LLC may take photos and videos of your pet while your pet is receiving services. I hereby grant Furr'd by T. Reid, LLC, and its affiliates the perpetual, irrevocable, royalty-free right and license to publish, distribute, adapt, modify, or otherwise use the Photo, or any portion thereof, in any manner for any commercial or non-commercial purpose without notice to you/your review or approval.

 

 

With my signature below, I certify that I have read and understood the agreement and waivers. I agree to abide by the regulations and accept all terms and conditions as set out.

First and foremost, the safety and well-being of your pet(s) are of the highest importance. Ensuring that your pet remains safe and well cared for is our first responsibility and as such, we take it very seriously. We do our best to have our pet parents screen for pre-existing health conditions, but some factors may be beyond our control. If a medical emergency arises while a pet is boarded by Furr'd by T. Reid, LLC it is imperative that we are immediately able to get them medical treatment at the closest available facility. We will call ahead to the veterinary offices in the closest proximity geographically to us to insure they can handle the emergency present. Your pet will be rushed to the closest available facility for treatment, and you will be notified. We notify the owner after we have secured a medical treatment center for the animal to avoid delays that may be caused by emotion on the part of the owner. Our goal is to get your pet's medical attention as quickly as humanly possible, and any distractions may interfere with that process. For that reason. it is a requirement to have our pet parents sign this form.

I understand that in the event of a medical emergency that Furr'd by T. Reid, LLC at its sole discretion deems to need the immediate attention of a licensed veterinarian. I authorize Furr'd by T. Reid, LLC to seek medical attention at the closest available veterinary facility. I further agree that I am financially responsible for any medical treatment my pet(s) receives as a result of a medical emergency while attending services provided by Furr'd by T. Reid, LLC.
