Authorization for Transportation: I hereby grant permission to agents of Tailored Security Services to transport me or my ward for purposes requested by and for me or my ward for personal trips and/or non emergency medical transportation.

I will not hold Tailored Security Services, contractors or volunteers responsible for unforeseen traffic incidents or interruptions in transportation should they arise.

I understand that once my trip is booked the fees are non refundable. No exceptions.

All representatives of Tailored Security Services will provide transportation in a vehicle that is considered safe and accommodating to the needs and comfort of the individual. All traffic laws will be abided by, which include seatbelt use for all passengers while the vehicle is in motion.
