    I am at least 18 years of age and have presented identification to the tattoo artist for verification.

    I am not under the influence of any type of drugs or alcohol. 

    I am not pregnant or nursing.

    I understand a tattoo is a permanent change to my appearance and obtaining a tattoo is by my choice alone. 

    I understand there is the possibility of an allergic reaction (or resulting infection) to the pigments (ink), soap, witch hazel, lavender, lidocaine, adhesive, or other items/processes used in my tattoo. 

    The tattoo artist or Tattoo 34 is not responsible for the meaning or spelling of a symbol or text provided for a tattoo. 

    I understand that aftercare, lifestyle, and maintenance will effect my long term results. I also understand that if I have any skin treatments, laser hair removal, plastic surgery, injections, or other skin altering procedures, may result in adverse changes to my tattoo.

    I understand there are security cameras in public areas of the shop. Footage is only reviewed to address any concerns (we haven't had any).

    I agree to these statements entirely of free will and sound mind.

  • You indicated you will be getting a hand/finger tattoo. 

    Compared to other parts of the body hands are washed and exposed to the elements more frequently, along with having lots of constantly moving parts. Hands also shed skin cells faster than other areas of the body. All of that causes the hands to be a very tough place to heal. Even in the best of circumstances hand tattoos may fall out (not stay) or not stay well and the ink tends to expand (bleed) under the thin skin of fingers and hands. We often see client expectations, not matching the results. Hand tattoos are an at your own risk type of situation, we will do them but we do not offer free touch up's on hand tattoos.

    By signing below you are agreeing that you understand the unique challenges of hand tattoos and are aware of the possibility that yours can expand, fade, or disappear completely. You also understand we do not offer free touch up's on hand tattoos. They can be touched up and/or redone but at your own expense.
