RETURN DEADLINE:

·  Single Night Home Sleep Apnea Testing kit is disposable and does not have to be returned after patient does study. However, if you don't do test, you have to return unopened and undamaged kit within 7 calender days to avoid $500 fine.

- No drop-offs are allowed. 

- Unused, unopened and undamaged Home Sleep Apnea Test (HSAT) kit must be mailed back in a padded envelope to the following address:

  1250 Borregas Ave, Ste. 62

  Sunnyvale, CA 94089 

- Please note that above address is NOT a drop-off location. 

- This fees will be solely your responsibility as it can't be billed to insurance.

-You will be responsible for the return shipping charges.

CONDITION OF RETURN:

·  The kit should be returned in good condition and include all original parts and packaging. 

·  Missing parts, visible damage to the kit or loss of kit will result in $500 fine. 

PAYMENT OF FINES:

·  If you owe processing fees, your credit card on file will be charged within 2 days of fine due. Unpaid fines may result in additional fees or be sent to collections.

- The fees will NOT BE WAIVED ONCE INCURRED.
