Pursuant to the Real Estate Act of Alberta, you are hereby notified that Martello Management (AB) Inc., is an agent of the property owner ("Owner") and shall act in the best interest of the Owner at all times. Martello Management (AB) Inc., cannot represent the Applicant, provide advice to the Applicant or act on the Applicant's behalf.

I ACKNOWLEDGE, AGREE AND CONSENT TO THE FOLLOWING:

For the purposes of assessing the Applicant’s current and ongoing eligibility for tenancy, the Applicant hereby consents to the property owner “Owner” and The Martello Group and/or one of their affiliates or subsidiaries obtaining and viewing personal information of the Applicant, including but not limited to:

Credit information and reports including credit scores;
Tenant records and rental history information;
Previous landlord references;
Personal references;
Employer references;
Employment and income verification information;
Personal identification information.  

The Applicant’s consent is deemed effective as of the date of this Application and will be valid for as long as required to fulfill the purposes described herein.
