Before You Begin A valid government-issued photo ID is required for all permanent makeup, tattoo, and removal services. You will be asked to upload your driver's license or identification card during this form. If you experience difficulty uploading your ID, please plan to arrive early and complete your paperwork in the studio.
Arrival Policy If you arrive early, please text the studio and remain in your vehicle until your appointment time unless instructed otherwise. If you need assistance with paperwork, please let us know. If you have not heard from us within 10 minutes of your appointment time, please text or call 502-906-6088.

Installment Plans & Financing

I understand that The Brow Babes offers installment plans that allow the total cost of a service to be divided into smaller payments over time.
TOTAL AMOUNT DUE I understand that the total amount due remains the same regardless of whether I pay in full, use an installment plan, or utilize third-party financing.
BOOKING FEE I understand that booking fees are non-refundable. If a booking fee is waived as part of a promotion, financing incentive, model opportunity, special offer, or courtesy, the waived booking fee becomes immediately due if I cancel, no-show, forfeit my appointment, or reschedule outside of policy.
I understand that installment plans are for the total service purchased, not individual appointments. Choosing not to attend future appointments does not eliminate any remaining balance owed.
I understand that financing obtained through Affirm, Cherry, Klarna, Afterpay, CareCredit, or any third-party lender is a separate agreement with that company and cannot be canceled, modified, forgiven, or refunded by The Brow Babes.
I understand that financing is a payment method and does not make a service refundable. Once financing has been approved and processed, funds are paid to The Brow Babes and are not eligible for refunds due to cancellations, rescheduling, no-shows, scheduling conflicts, dissatisfaction, inability to complete treatment, or failure to attend appointments.
I understand that financing agreements remain active regardless of attendance, cancellation, rescheduling, or no-show, and that all fees are based on the value of the appointment reserved, not any remaining balance owed.
Rescheduling & Cancellations

RESCHEDULING, CANCELLATIONS, NO-SHOWS & FINANCING Please notify us at 502-906-6088 if you need to cancel or reschedule your appointment.
I understand that Sundays, Mondays, holidays, and studio closure days do not count toward cancellation or rescheduling notice requirements. Requests are processed during business hours, and messages sent while the studio is closed are considered received on the next business day.
Example: If my appointment is Tuesday at 10:30 AM and I text on Monday while the studio is closed, my request will be considered received on Tuesday and may be subject to same-day rescheduling fees.
RESCHEDULING FEES More than 72 business hours:• One complimentary reschedule 48–72 business hours:• $50 rescheduling fee 24–48 business hours:• $75 rescheduling fee Less than 24 business hours:• $100 rescheduling fee per service
SAME-DAY CANCELLATIONS & SAME-DAY RESCHEDULES Because same-day appointment slots cannot typically be filled, clients are responsible for 50% of the reserved appointment value plus the applicable rescheduling fee before a new appointment may be scheduled.
I understand that same-day cancellations and same-day reschedules are charged based on the value of the appointment reserved, not the remaining balance owed, amount financed, installment balance, or payment method used.Examples:• $299 appointment = $149.50 (50%) + $100 rescheduling fee = $249.50 due• $530 appointment = $265 (50%) + $100 rescheduling fee = $365 due• $1,060 Brow + Lip appointment = $530 (50%) + $100 rescheduling fee = $630 due
NO-SHOW APPOINTMENTS Clients who fail to attend their appointment without notice are responsible for 100% of the reserved appointment value.Clients who no-show may be required to begin the booking process again, pay a new booking fee, and satisfy any outstanding balances before future appointments can be scheduled.
☐ I understand that financing (Affirm, Cherry, Klarna, Afterpay, CareCredit, etc.) is a payment method for my service and does not reserve appointment time. Financed appointments are subject to the same cancellation, rescheduling, and no-show policies as all other appointments. My financing agreement remains active regardless of attendance, cancellation, rescheduling, or no-show, and all fees are calculated using the value of the appointment reserved, not the remaining balance owed.
INSTALLMENT PLANS Clients utilizing The Brow Babes installment structure remain responsible for the full service amount agreed upon at booking.Choosing not to attend a Perfecting Session, declining future appointments, or discontinuing treatment does not eliminate any remaining balance owed.Cancelling an appointment does not cancel a financing agreement or installment agreement.
LOSS OF PROMOTIONAL PRICING Late cancellations, same-day cancellations, no-shows, and excessive rescheduling may result in the loss of promotional pricing, discounts, waived booking fees, financing incentives, special offers, and other promotional benefits previously provided by The Brow Babes.
WEATHER, EMERGENCIES & STUDIO CLOSURES Reschedules or cancellations due to city, state, federal, or weather-related closures are not eligible for refunds or chargebacks.If appointments are affected by severe weather, illness, family emergencies, natural disasters, states of emergency, government-mandated closures, or other unforeseen circumstances, appointments will be rescheduled at the discretion of The Brow Babes. If JCPS is closed due to inclement weather, it is possible that the studio may also be closed. While this is not always the case, every effort will be made to provide proper notice.
OWNER DISCRETION We understand that genuine emergencies occur. The Brow Babes reserves the right to make exceptions in cases involving verified medical emergencies, hospitalization, military deployment, severe weather events, or other circumstances deemed appropriate by management.
I understand that appointments cancelled by The Brow Babes due to illness, emergencies, severe weather, family emergencies, or circumstances beyond the studio's control will be rescheduled and are not eligible for refunds or chargebacks.
Refunds & Chargebacks
LUXURY SERVICE, REFUND & LIABILITY ACKNOWLEDGMENT I understand that permanent makeup is an elective cosmetic procedure and is considered a luxury service.While every effort is made to achieve the desired outcome, I acknowledge that results vary from person to person. Factors such as skin type, lifestyle, aftercare, medications, health conditions, sun exposure, skincare products, and individual healing responses may affect color, retention, shape, symmetry, and longevity.No specific outcome, retention rate, color result, or healing result can be guaranteed.
REFUNDS, FINANCING & PAYMENT AGREEMENTS I understand that permanent makeup services are elective cosmetic procedures and are non-refundable once performed. Financing agreements through Affirm, Cherry, Klarna, Afterpay, or other third-party lenders are separate agreements between myself and the financing company and cannot be canceled, modified, forgiven, or refunded by The Brow Babes once processed.
CLIENT SATISFACTION If I have concerns regarding my service, I agree to contact The Brow Babes directly and allow a reasonable opportunity to review and address those concerns before pursuing chargebacks, payment disputes, complaints, or legal action.
CLIENT COMMUNICATION & DISPUTE RESOLUTIONI understand that unforeseen circumstances may occasionally require appointment adjustments, including illness, emergencies, severe weather, scheduling conflicts, double bookings, staffing issues, or other events beyond the studio's control. While The Brow Babes will make every reasonable effort to honor scheduled appointments, I acknowledge that changes may occasionally be necessary.I agree to communicate respectfully and contact The Brow Babes directly regarding any concerns related to my appointment, service, or experience. I agree to allow a reasonable opportunity for resolution before pursuing chargebacks, payment disputes, complaints, legal action, or other claims.I acknowledge that I have read, understood, and agreed to all booking, cancellation, rescheduling, payment, financing, and refund policies outlined in this agreement.
LIABILITY RELEASE I certify that I have disclosed all relevant medical information, medications, allergies, and conditions to the best of my knowledge. I understand the risks, limitations, healing variables, and potential complications associated with permanent makeup. I voluntarily consent to treatment and release The Brow Babes, its owner, employees, contractors, apprentices, and representatives from liability arising from known or unknown complications, healing outcomes, retention issues, adverse reactions, or failure to follow pre-care and aftercare instructions.
AUTHORIZATION I authorize The Brow Babes and its representatives to perform the permanent makeup procedure(s) selected by me and any related services reasonably necessary to complete the treatment.
Product & Retention Acknowledgement

Successful healed results depend on many factors including skin type, medications, health conditions, lifestyle, sun exposure, and proper aftercare. Failure to follow pre-care and aftercare instructions may negatively affect healed results and may require additional sessions at the client's expense.
I understand that healed results vary based on skin type, age, medications, health conditions, hormones, lifestyle, sun exposure, skincare products, aftercare compliance, and individual healing. No guarantees have been made regarding color, shape, symmetry, retention, healing, or final results. If I choose to proceed despite a known skin condition, I understand that results may be unpredictable and no refunds will be issued.
I understand that failure to follow pre-care and aftercare instructions may negatively affect healed results and may require additional touch-ups, color corrections, or future Color Boost appointments at my expense. Touch-ups are typically performed at 6–8 weeks, and Color Boost appointments are generally recommended every 12–18 months.
☐ I understand that photographic documentation may be required in order for The Brow Babes to evaluate concerns regarding healing, retention, color, shape, or results. Failure to provide requested photographs may prevent The Brow Babes from determining the cause of the concern and may limit available corrective options.
I understand that products such as Neutrogena Hydro Boost, retinoids, Retin-A, tretinoin, vitamin C, glycolic acid, lactic acid, salicylic acid, peptides, hyaluronic acid, anti-aging products, brightening products, chemical exfoliants, lash serums, and certain moisturizers may cause premature fading, poor retention, color shifting, or gray/ashy healed results. I understand that The Brow Babes is not responsible for pigment loss caused by these products.
Informed Consent of Possible Risks, Hazards, or Complications

By checking the I Understand box, you are initialing that you fully understand that you are entering a legally binding contract
I understand that permanent makeup is a cosmetic tattoo procedure that places pigment into the skin and may require multiple sessions to achieve the desired result.
I understand that The Brow Babes uses disposable instruments and follows strict sanitation and cross-contamination protocols.
I understand that all tattoo procedures carry risks including infection, pigment migration, uneven retention, fading, discoloration, scarring, bleeding, swelling, allergic reactions, and unsatisfactory healed results. No guarantee has been made regarding color, retention, longevity, or final outcome.
I understand that signs of infection may include excessive redness, swelling, itching, pain, warmth, unusual discharge, or oozing and that I should seek medical attention immediately if these symptoms occur.
I understand that if I am planning cosmetic surgery, medical procedures, laser treatments, or advanced skincare treatments, I should allow adequate healing time and wait at least 4 weeks before or after my permanent makeup procedure unless otherwise directed by my physician.
I understand that allergic reactions to pigments, topical products, numbing agents, and anesthetics are possible and may require medical treatment.
I understand that lip blushing may trigger cold sore outbreaks. I understand that The Brow Babes recommends consulting with my physician regarding antiviral medication prior to treatment. Unless otherwise directed by my physician, The Brow Babes recommends taking L-Lysine 1000mg daily and Valtrex 1000mg daily for 5 days before and 5 days after my lip blush procedure.
Spot & Patch Testing: If I am concerned about reactions to pigments, numbing agents, or products used during treatment, I may request a patch test at my own discretion. Patch testing does not guarantee that future reactions will not occur.
Lip Blushing & HSV-1 Disclaimer

Lip blushing can enhance your lip color and shape, but it may trigger HSV-1 (Herpes Simplex Virus) outbreaks due to the trauma to lip tissue.The procedure can cause trauma, potentially triggering an HSV-1 outbreak even if the virus has been dormant.

Preventive Measures:
- Take L-lysine supplements at least one week before and one week after the procedure.
- Consult your healthcare provider about taking antiviral medications like Valtrex (valacyclovir).
Access to Medication:

If you do not have a healthcare provider, you can request antiviral medications from ForHers or Hello Wisp
Touch Ups & Color Boosts
I understand that touch-up appointments are typically completed 6–8 weeks after my initial session. Permanent makeup naturally fades over time and future Color Boost appointments may be recommended to maintain color and definition. Appointments completed after the touch-up window may be subject to current Color Boost pricing.
I understand that healing and retention vary based on skin type, lifestyle, medications, and aftercare. If retention is poor due to improper aftercare or if additional sessions are needed beyond the included touch-up, additional charges may apply.
Right to Refuse Service & Studio Safety

Though it is extremely rare, we reserve the right to refuse treatment for the following reasons.
RIGHT TO REFUSE SERVICE I understand that The Brow Babes reserves the right to refuse or discontinue treatment without refund if I: • Arrive under the influence of drugs or alcohol • Have undisclosed medical or skin conditions that make treatment unsafe • Fail to follow studio policies or artist instructions• Display abusive, disruptive, threatening, or inappropriate behavior • Create an unsafe environment for staff or other clients
I understand that if I have had a fever within the previous 72 hours or arrive with a fever, my appointment may be cancelled or rescheduled for safety reasons.
I understand that documented emergencies such as hospitalization, surgery, emergency medical treatment, severe illness, or other qualifying circumstances may be considered for a one-time courtesy waiver at the studio's discretion.
I understand that photographs and videos may be taken before, during, and after treatment for documentation, training, quality control, marketing, social media, website content, and other business purposes. I grant The Brow Babes permission to use these images without compensation.
