Participation Waiver And Photo Release Form

I am aware of the risk of attending this event on this particular date. I promise not to hold the company, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves the company of any duty for injuries sustained to myself or any minors accomany to me and under my supervision on the premises before to, during, or after the activity.

By signing this agreement, I agree to hold the company completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.
I will do all in my power to obey corporate staff and all safety requirements, and I will seek clarification if necessary.

I grant Type 1 Unbound the right to take, edit, alter, copy, exhibit, publish, distribute, and make use of any and all pictures or video taken of myself and Minor(s) who accompany me to this event under my supervision to be used in and/or for any lawful promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits, and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration.
