    I understand that my child will be participating in activities that may involve risk. I understand that the UMA centre, its staff, and volunteers are not liable for any accidents, injuries, loss of property, damages or theft related to my child’s participation in the Youth Program. I also understand that disrespectful or aggressive behaviour, rude language or inappropriate phone usage will not be tolerated, and may result in my child’s exclusion or removal from the program at the coordinator’s discretion. I acknowledge that my child may be using private transport at times and give them permission to use the transportation provided by the UMA centre and its staff. I acknowledge that there will be no refund issued if my child withdraws from the program after week 4 (except in extenuating circumstances) nor if he is expelled. I give permission for photographs taken on the day to be used for promotional purposes ONLY by the UMA.


    UMA Rules and Regulations
    • Teams must consist of no more than 6 registered participants
    • Age is restricted from 12-14 years
    • No swearing, fighting or arguing with others
    • Volunteers and staff must be respected and listened to at all times
    • No phones are allowed to be used during the program, other than in an emergency • Awra must be covered at all times
    • Participants must be on the premises at all times
    • Failure to show up for 3 weeks consecutively unexplained or 5 overall unexplained absences throughout the term will result in the replacement of the participants.
    • Participants are not allowed to access the canteen throughout the program
    • All UMA property must be respected and mustn’t be damaged.

    By accepting these rules and conditions I agree to not send my child/ren to the program if they have any signs of flu symptoms or symptoms of sickness. By accepting these conditions I also understand that my child/ren can be sent home in the event that they do show flu-like symptoms. Please note: The first set of uniform will be issued as part of the rego. If replacements or extras need to be issued, a fee will apply.
