  • Starting June, 2025: Dr Sam Oldfield will be leaving UMedical to change his career focus to Palliative Care. This unfortunately means that he will no longer be able to be your primary care provider.

    Making sure you have ongoing access to primary care following his departure your care will automatically be transferred to Dr Barbara Szopinski, whose office is a one minute walk from UMedical at 366 Bloor St. East.

    If you have selected - OPTION 1
    (Continue my care with Dr Sarah Salman at IMD Medical):

    • Check your mail: Complete the new re-enrolment form and send it back via the pre-paid envelope 

    • If you did not receive the enrollment form, please email or call the administrative team at IMD Medical to obtain the new roster forms

    • Your chart and medical records will be transferred to IMD (no action or cost required by patient) 

    If you have selected - OPTION 2
    (Find another family doctor closer to me before January 2024):

    • I would encourage you to look for a new family doctor as you would no longer be a patient at UMedical after 1st January 2024

    • If after this point you are still looking, you may contact
      • Health Care Connect (https://hcc3.hcc.moh.gov.on.ca/) via telephone or via an online form, who can help you with this
      • Health 811 (https://health811.ontario.ca/static/guest/find-a-service)

    • Once you are ready to transfer your medical records to your new doctor/clinic, you can visit www.umedical.ca for updates on where to obtain your medical records. Please note that there is an administrative cost associated to this. 

    • Please note the chart transfer is recommended but is optional
