A USTA League Team Confirmation & Agreement form is required for all USTA League teams. Captains need to complete the information listed below and sign the form in order for the team to be included in the match schedule. Captains need to acquire confirmation (verbal, email, or written) from your facility prior to submitting this Agreement.

Agreements must be submitted before midnight on May 8, 2026
