  • Staff Supervision and Support Form

  • The aim of this form is to give a formal platform with which to feedback to staff on performance. It is to be completed by the Survival Leader at a time agreed with the member of staff. It should be completed within the first 2 weeks of a staff member's employment.

    It is not necessary to observe the whole session, but the start of the activity should be observed so that the process from registration, through set up, to activity delivery can be seen. Once completed, the feedback should be given to the staff member, including sharing this form with them.

    If improvement is required in any area, the Survival Leader should provide support and recommendations about how the instructor could improve their performance.
