Photo Release Form

I, the undersigned, hereby grant United4Rare and CortiCheck permission to use my photograph, image, or likeness in any media, including but not limited to the Organization's website, social media platforms, promotional materials, and publications, without further consideration or compensation.

I understand that:

My photograph may be edited, copied, exhibited, or published for lawful promotional purposes.

I waive the right to inspect or approve the finished product wherein my likeness appears.

This authorization is perpetual, worldwide, and irrevocable unless otherwise specified in writing.

I confirm that I am at least 18 years of age, or, if under 18, this release is signed by my legal guardian.
