    This is a non-CME opportunity for your organization to host a private, focused lunch session with registered attendees of the Southern Utah Family Medicine Conference (SUFMC). This session is not eligible for continuing education credit and may concentrate on a specific product, service, or therapeutic area.

    Product Theater Includes


    •     Multi-purpose meeting room at Rocky Vista University – Utah Campus with AV capabilities, including monitors, speakers, and a podium.

    •     You may bring your own computer to present or submit presentation materials in advance for UAFP staff to display.

    •     Lunch provided for anticipated 50–60 attendees.

    •     A post-session attendee list from your Product Theater session.

    •     A list of all conference attendees who have opted in to having their information shared.

    •     Attendance for up to four (4) representatives of your organization at the conference.

    •     Table with two chairs, linens, and access to power.

    •     Logo placement and company profile on the conference app.

    •     Logo placement on the conference Bingo Card.

    Important Information


    •     All Product Theater sessions require prior approval by UAFP. Submission of this application does not guarantee acceptance.

    •     This is a non-CME/non-CE presentation. Continuing education credit is not offered for this session.

    •     The sponsoring organization is responsible for arranging its own speaker(s) and presentation logistics.

    •     All promotional and marketing materials referencing this Product Theater must be reviewed and approved by UAFP prior to distribution.

    •     Promotional materials may not imply that the Product Theater is an official activity of, or endorsed by, UAFP.

    Required Disclosure Statement: All promotional and marketing materials for this Product Theater must include the following statement in a prominent location: “This Product Theater is a promotional activity and is not approved for continuing education credit. The content of this session and opinions expressed by presenters are those of the sponsor or presenters and not of the Utah Academy of Family Physicians.”

    Payment & Deposit Terms
    •     Payment may be made by credit card or check.

    •     A convenience fee of approximately 4.5% is assessed for payments made by credit card. This fee can be avoided by paying via check.

    •     Credit card payments: a 50% deposit is required at checkout, with the balance due in full by October 1, 2026.

    •     Check payments: a 50% deposit is due within 30 days of registration, with the balance due in full by October 1, 2026.

    •     Submitting this application does not guarantee acceptance. Sponsorship is confirmed only upon written acceptance by UAFP.

    Lodging

    A room block is available for conference attendees and vendors at the Black Desert Resort. A link to reserve rooms will be provided upon acceptance of this application. We ask that vendors reserve more than two rooms. Additional lodging recommendation in the St. George, Utah can be provided if requested.
