DECLARATION

I,willingly consent in sending my son / daughter to VHP Sanskrit Schoolto learn, practice and appreciate dharmic way of life and also to know more about Bharathiya heritage, develop social awareness and leadership skills to benefit self, community/country where we live in.

I give permission to my child to participate in the activities of VHP Sanskrit School. I understand that some of the activities in which my child may participate may involve a risk of physical harm. These activities include indoor and outdoor programs under the supervision of designated volunteers. Although the organization and the instructors will provide my child with appropriate directions and will endeavour to minimize risk of harm to my child, these inherent risks are beyond the control of Bala Samskar Kendra and its volunteers.

I am aware that some activities in which my child may participate carry with them risks of harm that are particular to that activity such as but not limited to:

Ø strain of muscles/ligament/cartilages                                                                           > insect bites

Ø collision with persons/obstacles causing injury                                                            > slipping/tripping

Ø falls due to undulating wet/slippery surfaces                                                                > foreign material/object entering eye/skin irritation

Ø being hit by an object at speed                                                                                     > being hit by swinging ruler/sporting equipment

Ø broken/dislocated bones or joints                                                                                  > injury by flora/fauna

Ø injury caused by burns/cuts/abrasions                                                                          > injury caused by atmosphere conditions.

In the event of illness or injury to my child while at school or an excursion, or travelling to or from school, I authorise the volunteers of Bala Samskar Kendra, if/where it is impossible to contact me, to consent to emergency medical treatment as is necessary by a qualified medical practitioner. I also undertake to pay or reimburse costs which may be incurred by Bala Samskar Kendra for medical attention, ambulance, transport and medicines.

I hereby release discharge and hold Bala Samskar Kendra harmless from any liability resulting from injuries suffered by my child arising out of participation in the classes. Further, I voluntarily waive all rights to bring legal action against Bala Samskar Kendra or its agents for any cause related to my child’s use of its program or facilities.
