  • Hi! Welcome to my practice. I look forward to working with you. Please take a moment to fill out an online intake form before your visit. All information is kept completely confidential.

    You agree that the information you have provided is to the best of your knowledge and true. You understand that this information will be used to help in treatment planning and counselling sessions.

    I authorize Sarah Warry Counselling and Addiction Recovery Services to collect my personal and medical information.  I also understand that my personal and medical information is confidential and will and can only be disclosed to third parties with my permission. I understand the only time my file will be shared without my consent is 1) In the event it is subpoena by a court which I will be made aware of, 2) Under " Duty To Report" Laws: If there is a threat of harm to myself or someone else, or there are children in my care who are in threat of danger.

    You will be required to sign a Release Of Information to have your information or file shared with anyone other than yourself.

  • Payment and Cancellations 

    I am aware of the Payment and Cancellation Policy.

    There is no fee for the initial 20 minute consultation. Each counselling session thereafter is $120.00 per hour, unless it is a couples/family session which is $150.00

    You are responsible for paying BEFORE the time of your session unless prior arrangements have been made. Payment must be made by E-Transfer to contact@sarahwarrycounselling.com.


    If there are ongoing non-payments or you refuse to pay your debt, I may close your file and legally seek to have the debts paid by a collection agency or other means.

    Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole that could have been filled by another client. As such, I require 24 hours notice for any cancellations or changes to your appointment. Clients who provide less than 24 hours notice, or miss their appointment, will be charged a cancellation fee of $60 for their first NO SHOW. There is a 15 minute window time given incase of lateness or troubles with connection during each session. If you do not show up after that time, it is assumed you are not attending the session without notice. Upon each follwoing no show, $120.00 late/ cancellation fee is applied for lost time and administrative fees.

    I also ask you please do not show up to your appointment under the influence as you will not be seen, but will be invoiced. Please be in a private area where confidentially and privacy will not be breached. If you may be in danger during our session, please arrange a SAFE WORD OR GESTURE with me prior to the session indicating you want 911 to be called when mentioned or gestured.

  • Risks and Benefits of Counselling and Expected Outcomes

    Counselling has both benefits and risks. Risks may include experiencing uncomfortable feelings such as sadness, shame, guilt, anxiety, anger, frustration, loneliness and helplessness. The process of counselling often requires discussing the unpleasant aspects and traumatic experiences that have occurred in your life. However, counselling has been shown to have benefits for individuals who undertake it. Counselling often leads to a significant reduction in feelings of distress, increased satisfaction in interpersonal relationships, greater personal awareness and insight, increased skills for managing stress and resolutions to specific problems. While this sounds promising, there are no guarantees about what will happen, and how much change will be made in your life.

    Counselling requires a very active effort on your part, and in order to be the most successful, you will have to work on things we discuss outside of our sessions in your day-to-day life. Recovery is often a life long practice and requires ongoing work and dedication to staying healthy.

    I understand that my counsellor can not give me a specific timeframe which I will be successfully finished counselling. I also understand that my success will highly depend on my commitment to do my counselling work both in and outside of counselling, and how much I utilize what I've worked on in counselling in my actual life.
