Under FERPA, students have the right to inspect their education records upon written request. Students who would like to view a copy of their education records must complete the request form below.

This form is used only to view copies of one's records. To submit your records to a third-party, please use the Authorization for Release of Information form.

(For individuals with power of attorney, or survivors of a deceased student, please complete only the lower section of this form.)
