Verification of Deployment/Service Form
The George and Helen Bennett Fund for Missionary Service has been established to remove student loan debt as an obstacle for Wheaton College students and alumni called by God to serve on the international mission field. This Fund provides for the refinancing of the educational debt of Wheaton College alumni who are preparing for mission service and are ready to proceed to the field. Recipients of the Bennett Fund will be required to serve a minimum of four years with 25% of the total loan debt being forgiven for each year of field service.

The purpose of this form is to verify that the recipient of the Bennett Fund for Missionary Service (“Recipient”) is serving overseas in the mission field. This form must be completed and signed by the Recipient’s supervisor.

1. The information is true to the best of his/her knowlege,
2. A Verification of Service Form will be submitted every 12 months until the recipient has earned 100% loan forgiveness (a total of 48 months of service), and
3. Wheaton College Student Financial Services will be informed of any change in the recipient's status with the sending organization.
