WORSHIP + WORKOUT – ACCIDENT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT Entities Covered: Worship + Workout, its organizers, volunteers, instructors, guest leaders, sponsors, and all other persons, firms, corporations, associations, employees, or partnerships involved with this event.
PLEASE READ CAREFULLY – THIS AFFECTS YOUR LEGAL RIGHTS
In consideration of being permitted to participate in Worship + Workout (the “Activity”), I hereby acknowledge and agree as follows: 1. ASSUMPTION OF RISK I understand that participation in the Activity involves physical movement and exercise that carries the risk of serious injury, disability, or death. These risks may result from my own actions or inactions, the actions or inactions of others, the condition of equipment or facilities, weather conditions, terrain, traffic, and other risks inherent to physical activity. I voluntarily assume all such risks. 2. PHYSICAL FITNESS CERTIFICATION I certify that I am physically fit, capable of participating in physical activity, and have not been advised otherwise by a qualified medical professional. I acknowledge that I am solely responsible for monitoring my own physical condition and stopping participation if I believe it unsafe. 3. WAIVER AND RELEASE I hereby WAIVE, RELEASE, AND DISCHARGE Worship + Workout and all entities and persons named above (collectively, the “Releasees”) from any and all liability for any injury, property damage, loss, or death arising from my participation in the Activity, whether caused by the negligence of the Releasees or otherwise, including while traveling to and from the event. This release applies to all claims, actions, demands, liabilities, costs, or expenses of any kind. 4. INDEMNIFICATION I agree to hold harmless and indemnify the Releasees from all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of my participation in or connection with the Activity. 5. MEDIA ACKNOWLEDGMENT I understand that by attending this event, I may be photographed, videotaped, or recorded. I agree that Worship + Workout and the Releasees may use my image or likeness for promotional purposes without compensation or further notice. 6. 18+ EVENT AND MINOR POLICY This event is strictly 18 years of age or older. I certify that I am at least 18 years old. I understand that the Releasees are not responsible for any minors or children who may be present. Any minor present must be supervised by an adult who assumes full responsibility for them, including any media capture. 7. CONSENT TO MEDICAL TREATMENT In the event of injury, accident, or illness, I consent to receive medical treatment deemed advisable. I understand that I am responsible for any medical expenses incurred. 8. COVID‑19 ACKNOWLEDGMENT (Optional) I acknowledge the ongoing risks associated with COVID‑19. I affirm that I and my household members do not currently have, nor have experienced within the last 14 days, symptoms associated with COVID‑19 (e.g., fever, cough, difficulty breathing). I understand that Worship + Workout and the Releasees cannot be held liable for exposure caused by inaccurate information provided. 9. ACKNOWLEDGMENT OF UNDERSTANDING I have read this document in its entirety and fully understand its contents, including that I am giving up substantial rights, including my right to sue. I am signing this agreement voluntarily and of my own free will.

I, the undersigned participant, affirm that I am of the age of 18 years or older, and that I am freely signing this agreement. I certify that I have read this agreement, that I fully understand its content and that this release cannot be modified orally. I am aware that this is a release of liability and a contract and that I am signing it of my own free will.
