Declaration Domestic Partnership Notary assignment completed.
You will receive the Declaration Domestic Partnership (DDP) application by mail, you will need to locate a notary in your area. 
DO NOT SIGN YOUR NAME ON THE DDP APPLICATION. Wait and locate a notary in your area.  Once you locate a notary, he or she will then ask you  to sign your DDP application.
He or she will notarize your signature on the Blank Notary Acknowledgement Certificate that I provided to you.

You can Walk-in your DDP application paperwork (DDP application and both Notarized Acknowledgement Certificates with the fee) to the Los Angeles office or Sacramento office or you can mail your DDP application and both Notarized Acknowledgement Certificates with the fee (processing fee + fee for each certificate) to the California Secretary of State in Sacramento. 

 

You can walk in the paperwork in either the Los Angeles office or the Sacramento Office. Some, not all of my clients have received their paperwork within the hour when they walked it in. 

If you mail the paperwork to Sacramento, it can take up to 10weeks to get your paperwork back.

You Will Receive After It Is Registered and Recorded By The Secretary of California. It will take up to 12 weeks to receive back from Sacramento.

These Original Documents That You Receive From The Secretary of State Must Be Mailed To The Prison Family Visiting Officer. Make Sure You Mail All Pages (5pgs) To The Prison Family Visiting Officer. Including The One Page That Has The Rubber Seal On The Back Of The Page.

After you mail the documents that you receive from the Secretary of State to the Family Visiting Officer. The Family Visiting Officer will have your documents on file.

If You Need To, Make Photo Copies For You To Keep.

Make photo copies of all the paperwork that you received from Sacramento.

 

If you pay the $43.00 to the Secretary of State, you will receive two certified certificates.
One original certificate will need to be mailed to the Prison Visiting Officer.
The other original certificate you receive, you will keep with the photo copies of the paperwork you received. This is only just in case you need it.
