  • Hospital, Convalescent and Nursing Homes notary mobile services. We provide essential services for individuals in hospitals, nursing homes, hospices, and rehabilitation centers. We are professionals who will travel to medical facilities to notarize critical documents like Power of Attorney, advanced directives, and healthcare directives for patients.

    The signer must be alert, mentally competent, and have valid non-expired ID.

  • I Do Not Provide or Offer Free (Pro Bono) Services. If you need my services, there is a service fee that must be paid to hire my service.

    Handle sensitive legal documents, including Advanced Healthcare Directives, Power of Attorneys, and Last Will and Testament, directly in patient rooms.

    Availability: Services are often available 7 days a week, including holidays and weekends, to accommodate urgent medical situations.

    Requirements:
    Patient Capacity: The patient must be lucid and understand what they are signing.

    Identification: Patient must have a valid (non-expired) government issued photo ID is required.

    Witnesses: Often, witnesses are needed in addition to the notary, and staff cannot always serve as witnesses. Witnesses who are signing, must also have a valid (non-expired) government issued photo ID.

  • To ensure a legal and valid notarization in a medical setting, the following conditions must be met:

    Alertness & Awareness:

    The signer must be lucid and fully understand what they are signing.

    No Medication Impairment:

    Signers should not be under the heavy influence of psychoactive medications or painkillers that inhibit cognitive awareness.

    Willingness:

    The notary must verify the signer is acting of their own free will and is not being coerced by family or others in the room.

    IMPORTANT- THIS IS A MUST…
    The individual signing the POA must be able to understand what’s going on and must be able to sign the POA document without assistance. If he or she can not sign his or her name and can only make an "X", then I will need 2 Witnesses (not medical care workers) friend or family members or acquaintancesn.

    A signature by mark is an alternative to writing a full signature. The signer instead makes a mark (such as an "X") or other symbols on the document.

    It is a practice to have one of the witnesses print the signer's name next to the mark on the document, and the witnesses will be required to sign the Notary’s journal entry.
    The second witness cannot be the person being assigned as the agent (attorney in fact)
