1. Program Participation
For my child to participate in the Bellevue Academy of Arts & Music programs at the following locations:
Bellevue Academy of Arts & Music, 12356 Northup Way, Suite 103, Bellevue WA 98005, and its buildings and all businesses associated with the North Park Business Center at 12328–12372 Northup Way, Bellevue WA 98005
I hereby waive, release, and discharge any and all claims for damages for personal injury or property damage that may occur as a result of participation in said programs.
This release is intended to discharge in advance the venues of Bellevue Academy of Arts & Music, teachers, employees, family members, volunteers, and peers (students) from liability, even if such liability arises from perceived negligence. It is understood that some activities involve an element of risk, and knowing those risks, I hereby assume them. This waiver, release, and assumption of risk is binding on my heirs and assignees.
2. Conduct and Withdrawal
It is understood that my child must abide by the rules and codes of conduct at all program venues. If the child exhibits excessive aggression or disruptive behavior, program staff reserve the right to dismiss the child without refund.
Important Disclosure:
Students may be withdrawn from the program for any of the following:
Unresponsiveness to instruction: Demonstrating one or fewer active responses out of five continuous prompts by the end of the second class, while over 90% of other students follow prompts.
Disruptive behavior: Significantly impeding class (pauses longer than 30 seconds per occurrence, up to three times per class, due to verbal, visual, or physical actions, such as screaming, pounding instruments, running, talking over the instructor, throwing items, or repeated behaviors). Parents of students with learning disabilities should consult staff to develop the best approach.
Inability to follow instructions or performing the opposite action when prompts are imperative to the curriculum.
Program staff reserve the right to terminate enrollment for misconduct, including but not limited to:
- Physical contact or abuse toward others
- Verbal abuse or use of foul language
- Threats or intimidation
- Tampering with or damaging another’s property
- Unsanitary behavior
- Use, sharing, or coercing others to use inappropriate substances
All incidents will be documented in a detailed case report.
3. Drop-Off and Pick-Up Procedure
All drop-offs and pick-ups are managed through the BAAM Self Check-In app. Upon arrival, open the app using the QR code posted at the entrance or visit form.bellevueacademy.com/checkin and complete the appropriate drop-off or pick-up category. For drop-offs, you will be called when we are ready for you. For pick-ups, complete check-out in the app and wait in the lobby until your student is brought out to you. For students ages 3 to 5, a parent or guardian must remain in the lobby for the duration of the visit.
4. Medical Consent
If the child is a minor, the undersigned parent or guardian authorizes Bellevue Academy of Arts & Music to provide emergency medical treatment as deemed necessary while participating in program activities at the listed venues.
5. Consent to Share Work as Data
By participating in BAAM programs, which include MathPiano®, PianoTowers™, and other research programs, you agree that all work created as part of participation—including recordings, compositions, assessments, and other materials—may be collected and used by BAAM for educational, research, and program development purposes.
Collected data may be analyzed to:
- Assess student progress
- Improve teaching methodologies
- Develop new learning tools
- BAAM may use aggregated, anonymized data for research studies, presentations, publications, and other academic or educational activities.
Personal identifying information, including full name, address, or other sensitive details, will not be shared. Limited information, such as first name, age, and assessment results, may be publicly disclosed solely to demonstrate program outcomes and student achievements.
6. Use and Return of Materials
For students enrolled in general music programs, all MathPiano® and PianoTowers™ materials are copyrighted original works and must remain at BAAM. These materials, whether physical or digital, may not be taken home, retained, copied, or distributed.
Upon termination of enrollment, all materials must remain with BAAM. Unauthorized possession or duplication of MathPiano® or PianoTowers™ materials may result in legal action or other remedies deemed necessary by BAAM.
7. Daily Conduct, Facilities, and Personal Items
Bathroom and Water Fountain: Parents and guardians of students ages 3 to 6 are required to take their child to the bathroom before each class session begins. The bathroom is located approximately 10 feet along the corridor on the east side of the building. Bathroom and water fountain requests after class has begun disrupt the learning experience of all students.
Students ages 3 to 5 enrolled in General Music classes: a parent or guardian must remain on BAAM premises, in the lobby or designated waiting area, for the full duration of every class session. In the event that a student requires bathroom assistance during class, the parent or guardian will be called immediately and is expected to respond promptly. If a parent or guardian is not reachable and a staff member must accompany the student, this constitutes a failure of parental support obligation. A second occurrence in the same enrollment period will be formally noted. A third occurrence will result in non-renewal of enrollment, as it represents a breach of the parental support responsibilities agreed to under this enrollment contract.
Students ages 6 and above use the bathroom and water fountain independently using the classroom hall pass. The water fountain is located near the bathroom on the east corridor and is subject to the same hall pass requirement.
Hydration: Students are encouraged to bring a sealed water bottle to each session. Water bottles must be left at the reception desk upon arrival, clearly labeled with the student name. Only sealed tumblers or bottles containing plain water are permitted. Juice, carbonated beverages, energy drinks, open containers, and sugary drinks are not permitted on BAAM premises. Students without a water bottle may use the water fountain with the hall pass. For students ages 3 to 5, a parent or guardian from the lobby must accompany the child.
Food in Classrooms: No food or drink of any kind is permitted in any classroom, studio, lesson room, or practice room.
Personal Items: Students are not permitted to bring personal items, toys, games, books, craft materials, or any belongings not required for their enrolled program into any session. Items that are not program materials must be stored at the reception desk upon arrival and may be retrieved at the end of the session. BAAM is not responsible for personal items left on premises.
8. Attendance, Sign-In/Out, Absence and Make Up

Refunds or fee adjustments are not provided for missed classes, absences, or scheduled program breaks. Make-up classes may be arranged in alternative classes, subject to availability.
9. Consent for Communication
By participating in the BAAM program, you consent Bellevue Academy of Arts & Music to contact you via email and/or text message for enquiry responses, new intake updates, class availability, registration openings, academy news, events and relevant promotions. You may withdraw your consent at any time by contacting academy administration.
