    Player Information Player's Full Name:

  •  All players should bring their own ball appropriate size for their age group, shin guards and appropriate soccer shoes and cleats.

    This registration contract is for 2025 summer/falls player fee is $130 per month, includes 1 hour practice twice a week. For the summer season

    WINTER SEASON  player fee is $130 per month and only one time payment of $200 for player , includes 1 hour practice twice a week .

    This fee does not include Uniforms, Jerseys and Tournaments games. Cipriani Soccer Academy is focused on training and soccer practices. Cipriani Soccer Academy will participate in games or tournaments as directed by the Academy. All Players will receive training from a coach from the Cipriani Soccer Academy.

    Registration Fee Payments are due by the 5th of each month. A late fee of $20 will be charged after the 10th of each month to the installments. If you are having significant challenges with staying up to date with registration fees, please notify Cipriani Soccer Academy. Registration installments fees are not refundable. Your registration is a commitment to participate. Fees for participation in Cipriani Soccer Academy cover the entire Spring/Summer season. A player who accepts registration to play with Cipriani Soccer Academy commits to pay the monthly/ entire fee for the full season. No refunds, partial or full, will be made to players who choose not to participate at any point after registration for any reason. If a player is injured during the season and cannot play or practice for the remainder of the season notify Cipriani Soccer Academy.

    Field Closures/Cancellations: Cipriani Soccer Academy will not be held responsible for any cancelled games or practices due to field closures. Cipriani Soccer Academy and its coaching staff will make every effort to make- up practice sessions that have been canceled due to inclement weather or other reasons, subject to filed and coach's availability.

    Registration Agreement I hereby authorize. My child has received a physical examination by a physician and has been found physically capable of participation in the soccer program such as soccer practices and games and have no illness, disease. existing injury. or disability that would be aggravated by my participation. I will inform Cipriani Soccer Academy if the status changes. By signing below, I agree with the registration agreement, and I am granting permission to allow my child to participate the Cipriani Soccer Academy. I agree and understand to be responsible financially for the reasonable cost of assistance by the 5th of each month.

  • Waiver and release of Liability and Notice of Risk Disclaimer of Liability

    Soccer is a physical, contact. sport that involves the risk of injury or illness can and may occur. I hereby authorize Cipriani Soccer Academy to seek and consent to emergency medical attention for my child as needed; and I further agree to be liable for the responsibility to pay all costs incurred in connection with such medical attention treatment. Cipriani Soccer Academy does not have personal injury insurance that covers my participation. Therefore. I should have a current, active personal injury insurance policy in force which, covers my participation. Under any condition, I am responsible for all medical expenses arising from my participation both in practices and games and while travelling to and from these events. I hereby acknowledge that participation in risk-oriented programs and activities involves an inherent risk of physical injury and I assume all risks.

    I hereby release waive liability. discharge, hold harmless, and covenant not to sue Cipriani Soccer Academy employees, coaches, trainers, associated personnel volunteers against any claim from all liability incurred in the conduct of, and participation, demands, causes of action and possible causesof action related to any loss, damage, illness, or injury that may be sustained by my child while participating in this soccer program.

    1. Activities associated with Cipriani Soccer Academy can at times involve substantial risk of injury, property damage, and other dangers associated with participation in Cipriani Soccer Academy. Dangers peculiar to such activities include, but are not limited to: broken bones, sprains, bruises, concussion, wounds, heat exhaustion, cuts, heat stroke, short of breathiness, burns, and any related injuries with muscles and tendons of any kind.

    2. Each participant, volunteer, and staff of Cipriani Soccer Academy should realize that there are inherent risks, hazards, and dangers involved including those mentioned in "Section 1". It is the responsibility of each person involved to engage only in those activities from which she/he has the tasks to do it.

    3. Everyone has the responsibility to read the above notice carefully and acknowledge the information of this document fully understands and acknowledge that there are risks and dangers associated with participation in any Cipriani Soccer Academy events and activities which could result in bodily injury, partial and/or total disability, paralysis, and death. I have completely read this document and fully understand its contents. As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and release for, and on behalf of, the participant (player/minor) named above. I hereby bind myself, the minor and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for. and on behalf of, the minor in the execution of this Waiver of Liability and Release.

    EXPECTATIONS

    1. Be prepared and on time for all practices and games.

    2. Give 100% effort in everything we do.

    3. Every player is expected to help set up for practices and games and pick up afterwards.

    PLAYER CODE OF CONDUCT

    1. Play the game for the game's sake, and not just to please my parents or coach. 2. Be modest and generous when I win and gracious when I lose.

    3. Work for the good of my team and always give my best effort.

    4. Show good sportsmanship before, during, and after games. I understand that soccer is a game, and that the players on the other team are my opponents, not my enemies.

    5. Conduct myself with honor and dignity and treat other players as I would like to be treated.

    6. Help my parents and fans understand the laws of the game so they can watch and enjoy the game better.

    7. Control my temper and not retaliate, even if I believe I have been wronged.

    8. Show respect for the authority of the referee, even though I will sometimes disagree with his/her calls.

    9. Profanity and bullying will not be tolerated in games and practices.

    10. Respect the game of soccer and play the game fairly.

    Player consequences will be determined by Cipriani Soccer Academy, for violations of the player Code of Conduct.

    EXCELLENCE

    1. No player ever becomes a success by giving up. Keep trying.

    2. We can't control the outcome of games. We can only be the best player, the best teammate, the best person we can be.

    AGREEMENT ON HOW LONG THEY PLAY

    Parents and players understand that coaches /assistant determine who plays and how often. The emphasis on competitive team lineup can result in uneven playing/practice time among participants, with occasions when some athletes are not given playing/practice time. The coach / assistant is the one who makes the often-difficult decision of who starts, who plays and how long the participants play/practice. When it comes to the frustration that comes from seeing your son or daughter not playing games, all supporters are expected to be respectful.

    24 HOUR RULE

    The 24hr rule is a rule that is in place to help keep peace between the coach and parents. The rule is simple. Parents are not allowed to approach the coach about issues until 24hrs after the competition. Reasons for the rule:

    1. To allow the parents to calm down after the issue has developed.

    2. Allow the coach to calm down after the competition.

  • 3. Give parents and coaches time to think about the problem.

    4. To prevent any unnecessary acts or scenes in front of players and other parent's 5. To stop any embarrassing moments in front of the Players or child

    6. The hope that time to think about the issues will help to solve the problem in a positive manner and the right resolution.

    Sometimes in the heat of the moment parents and coaches say things that they really don't mean. This can hurt the player or team because things where not handled correctly. The parent & Coach relationshipis important, but the overall decisions should be based on what is best for the team and players. The ruleis in place to help protect the players and team.

    PARENT CODE OF CONDUCT

    As a parent, you play a special role in the development of your daughter or son. and of his/her teammates. Your encouragement and good example will do more to ensure good sportsmanship and self-discipline than any other influence. The other team is the opponent, not the enemy, and should be treated with respect. While winning is important, playing well and fairly is the essence of the game.

    1.Support your child by giving encouragement and showing interest in his/her team. Help your child work toward skill improvement and good sportsmanship in every game. Teach your child that hard work and an honest effort are often more important than victory.

    2. BE SUPPORTIVE AND YOUR CHILD'S BIGGEST CHEERLEADER

    Children learn more by example than by criticism. Work to be a positive role model and reinforce positive behavior in others. Applaud good plays by others on your child's team as well as good plays by the opposing team. Do not criticize any child's performance from the sidelines. Accept the results of each game. Teach your child to be gracious in victory and to turn defeat into victory by learning and working toward improvement.

    3. DEMONSTRATE A POSITIVE ATTITUDE TOWARD YOUR OPPONENTS AND THEIR FAMILIES

    Opponents are not enemies. Take care to show good hospitality at home and to represent in a positive way when playing other programs. Never allow yourself to be drawn into a verbal disagreement with opposing parents or coaches.

    4. REMEMBER THAT YOUR CHILD WANTS TO HAVE FUN

    Children must establish their own goals to play the game for themselves. Take care not to impose unreasonable demands on your child. Let your children experience the fun of playing as well as the challenge of excelling. You and your child will both enjoy the game more if you put some emotional distance between yourself and the field or play.

    VIOLATIONS OF THE PARENT CODE OF CONDUCT

    Violations of the Parent Code of Conduct may result in:

    A Parent being suspended and/or removed from a game/tournament by a referee for misconduct. A Coach being suspended and/or removed from a game/tournament by a referee for a misconduct.

    A Member or Family could be released from the program.
