If you have received a confirmation from us for this visit that your dog is up to date on vaccinations, you do not need to fill out this form!
For all our services, we require the following vaccinations:
Rabies
DHLPPC (or complete puppy series)
Bordetella
Canine Influenza (H3N2/H3N8)
For Canine Influenza (CI), we are following current veterinary advice: initial dose, wait 2-3 weeks, get the booster, then wait 3 days for full immunity. A single dose annually will keep CI current. Please work out your schedule with your vet and feel free to ask us for more info.
If your vet is out of CI vaccine, please
contact us. We know of several vets in the area who have it in stock.
If your dog has had a reaction to a particular vaccine in the past, or if for other reasons you prefer not to vaccinate, we do accept veterinary proof of titers. Be advised that titers generally take longer than vaccinations, and they are often more expensive.
Dogs lacking any of the above 4 vaccinations (or titers) are NOT eligible for any of our services. Check with your vet to make sure, and please plan ahead!
