1. Would you be willing to permit a home check?
2. Do you understand that this form and the contract following bringing a puppy home, if proceeding to that stage, will act as a legal, binding document of agreement.
3. If you were at any point to be expecting a new baby during any phase of life of your Great Dane, would you feel the need to return him/her to the breeder for any reason? If so, why?

41. We require that all dogs purchased from Great Danes Of Flat Rock be spayed or neutered at the appropriate time. Will this be an issue?
42. Do you plan to have pet insurance or care credit ?If neither how do plan to pay for veterinarian care ? Note..Once again do you understand that large breed animals can be very expensive if sick or injured!!
43. If puppy we're to be injured or sick would you have a issue reaching out to us for advice ?
44. Unless of emergency, would you have a problem notifying us before euthanizing ?
45. Can you tell us more about your employment status? How long have you been employed there ?
46. Do you have an issue with bringing ID's when picking puppy up ?
47. If 6 training classes are provided with pet-co would you utilize that package within the year ?
