I understand that it is a criminal offence to dispose of any cat(s) where i have knowledge of the true owner. In the event of a false declaration, Cat Haven will provide the information contained in this document to the relevant authorities.

I understand that Cat Haven is now responsible for any decisions relating to the welfare of this cat(s), including re-homing, or euthanasia. In addition, I am aware that no further discussion will be entered into and that the outcome relating to the cat's welfare will not be made available to me.

Please be aware that this cat(s) could be euthanised today or at the end of any required hold period.
