RADD application

Foster Agreement: RADD requires a $140 security deposit if you request a dog to be tagged to foster. Once the foster dog is adopted we will reimburse this $140 deposit. If the foster family decides to adopt this will go toward the adoption fee.

Foster Care: Dogs rescued by RADD remain under the rescue’s care until adopted. Any changes of location and medical care must be discussed with the RADD team.

We have no location to house dogs and rely exclusively on fosters as a safe haven for death row dogs. Our mission is to helpthese dogs feel safe and learn how to be part of a family. Some dogs arrive with very little or no traumaand others have had extremely negative interactions with other animals and humans. We ask ourfosters to keep in mind the trauma these dogs have experienced and to allow them the time todecompress (at least 3 days) and to learn to be part of your house (at least 3 weeks) before we start toassess their character. Just like humans thrust into a new situation, these dogs need time to adapt. Weare their second chance. Once we give the dogs a chance to feel safe, we can post their adoptioninformation for fosters. Whether adopting or fostering, you and your pup(s) need time to get to knowone another.Please sign and date here that you understand our mission._____________________________________________________________________________________*

1. Adopter/ Foster commits to administering preventatives - including heartworm, flea, and tick and ensures dog receives all age-appropriate vaccinations.

2. Adopter agrees to spay/neuter when age-appropriate. Proof of procedure must be emailed to rescue.

3. Adopter/ Foster recognizes the rescue endeavors to ensure dogs are healthy prior to transport, but underlying illnesses may be present or contracted during transport. The Adopter/Foster accepts this risk. This includes accepting risk of communicable diseases. The rescue bears no financial responsibility for this circumstance.

4. Adopter/ Foster accepts responsibility to appropriately integrate new dog into home. This includes slow introductions to resident pets.

5. Adopter/Foster commits to providing a 30 day notice for requests for new placement of the dog. While rescue will make every efforts to find new placement, it cannot be guaranteed.

I understand that completing this application does not guarantee I will be allowed to adopt a dog, and that the application is a tool to help RADD determine if this dog and my family are a good fit for each other. I acknowledge that all information provided on this form is true and correct. I understand that any misrepresentations may result in my not being approved and/or removal of the adopted dog from my possession. I also certify that I have never been convicted of animal cruelty or neglect.

I agree to allow a representative from RADD to contact my vet for a reference and also agree to allowing a home visit prior to adopting this dog. If I am approved to adopt a dog, I agree to pay a non-refundable adoption fee and hand sign an adoption contract at the time of adoption.
