  • Statement of Understanding
It is the responsibility of the licensed professional to be familiar with their scope of practice as it is defined by their licensing State.  It is the responsibility of the licensed professional to alert their medical malpractice carrier of their attendance and participation in any and all training opportunities.  It is the responsibility of the licensed professional to alert all supervising and delegating entities of their intentions to attend and participate in training opportunities, as well as provide supportive documentation of that awareness and approval.

Notice:  Participation in AAI training does not imply nor infer you are licensed to provide these services in your state.  Please check with your board for the regulations pertaining to the performance of these treatments. I understand and agree to check with my licensing board as to whether or not I am legally able to provide these services.  I will hold AAI harmless of any and all liability associated with my license and whether or not I am legally able to perform these services in my state.

    • COVID-19 Waiver of Liability
    • Waiver of Liability and Assumption of Risk Agreement
      COVID-19 Release Agreement

      Waiver:

In consideration of Aesthetic Advancements Institute ("AAI") allowing the undersigned to participate in an AAI live hands-on training course ("live training"), I for myself, my heirs, personal representative or assigns, do hereby release, waive, discharge, and covenant not to sue AAI, it’s owner, adminstrators, employees, agents and independent contracts from liability from any and all potential claims related to or resulting in personal injury or illnesses (including death) and property loss arising from any COVID-19 transmission that results from the undersigned being present at the facility and participating in training.

      Assumption of Risk:

I understand that AAI has put into place reasonable preventative measures aimed at reducing the risk of exposure to COVID-19 at the live training.  I also understand that due to the nature of this virus, there are certain inherent risks that cannot be eliminated regardless of the care taken to avoid COVID-19 transmission at the live training.  I understand that possible exposure to the COVID-19 virus may result in the following: a positive COVID-19 diagnosis, quarantine or self-isolation, hospitalization, ICU treatment, possible need for intubation, other potential complications, the risk of death, and other risks that may not be known at this time.

I assume all of these risks and hereby assert that my participation is voluntary and that I knowingly assume all such risks, and while aware of these risks, I have elected to be present at the facility and participate in the live training.

      Indemnification and Hold Harmless:

I also agree to indemnify and hold harmless AAI, its owner, administrators, employees, agents and independent contractors from any and all claims, actions, suits, costs, expenses, damages and liabilities including attorney fees resulting from any COVID-19 transmission that occurs from to being present at the facility and participating in the live training.

      Acknowledgement of Understanding:

I have read this waiver of liability, assumption of risk, and indemnity agreement, and fully understand its terms. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

    • Cancellation Policy Verification
    • Please read this policy in full and agree below.

Please note: All cancellations must be received (not postmarked) in writing, via email to admin@aestheticadvancements.com

A refund will be given, less a $150.00 administrative/processing fee if canceled 14 days prior to the start of the course. Any cancellation less than 14 days or "No Shows" will not qualify for a refund.  If we are within the 14 day period and we are able to find a "replacement participant", we will gladly switch your course fee to another applicable training date without penalty; however, if a replacement is not found the full amount of training will not be refunded.  Due to this policy, weather related cancellations are considered "No Shows" if the program is conducted.

As we do require a minimum number of registrants per each class, and if we do not meet this minimum (7-14 days prior to the training), or for any other reason beyond our control the course is canceled or postponed, the registration fee will be refunded in full.  Please note Aesthetic Advancements is not responsible for deposits to hotels, fees associated with necessary airline changes or non-refundable airline tickets.

Classes are filled on a first-come, first-served basis.  Some classes fill well in advance, so it is advised to register as soon as possible. We cannot reserve space in this program without receipt of registration packet and payment. You will receive written confirmation from our office when we receive your payment/registration.  If you have not received a confirmation within 10 days, please contact us.  Your registration is not finalized until you receive a confirmation from our staff. Confirmations are generally sent via the email address you provide on your registration packet.  Due to the limited space available in this class, we are unable to accommodate unregistered participants the day of training.

The injectable training sessions include a hands on portion.  The products in most courses will not be provided by the organization, therefore, the student is required to bring their own.  Products must be legally secured through an FDA approved product manufacturer.  Products secured from outside the US will not be permitted.  Students who bring products determined to be illegally secured will not be able to participate in the hands-on portion of the class.  Refunds will not be provided for any portion of the training. If you have further questions regarding FDA approved products and laws regarding products brought in from outside the US, please contact the FDA directly for additional clarification.

We ask that each registrant check with his/her State Licensing Board and review the practice act pertinent to their scope of practice.  We do require you attach a copy of your valid state medical or nursing license with your registration packet.  Please direct any "scope of practice" questions to a member of your state board, as we will be unable to answer questions pertaining to individual state legalities.
