Assumption of Responsibility:It is the responsibility of the licensed professional to be familiar with their scope of practice as it is defined by their licensing state. It is the responsibility of the licensed professional to alert their medical malpractice carrier of their attendance and participation in any and all training opportunities. It is the responsibility of the licensed professional to alert all supervising and delegating entities of their intentions to attend and actively participate in hands-on training opportunities, as well as provide supportive documentation of that awareness and approval if requested.
Notice: Participation in AAI training does not imply nor infer you are licensed to provide these services in your state. Please check with your state licensing board for the regulations pertaining to the performance of these treatments for both training and practice purposes.
Statement of Understanding:I understand and agree to check with my licensing board as to whether or not I am legally able to provide these services and what conditions are required in order to legally perform these procedures. I will hold AAI harmless of any and all liability associated with my license and whether or not I am legally able to perform these services in my state. I understand that I will be providing patient treatment during the hands-on portion of the training and I am responsible under my current state license for any treatments provided. I understand that it is my responsibility to acquire any necessary supplies, including but not limited to injectable products, anesthetics, patient model volunteers, as requested by Aesthetic Advancements Institute for utilization during the hands-on training whether through my own authority or through my supervising and delegating entities.
Acknowledgement of Understanding:I have read this assumption of responsibility and statement of understanding, and fully understand its terms. I acknowledge that I am signing this waiver freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to Aesthetic Advancements Institute to the greatest extent allowed by law.
I have agreed to participate in this interactive hands-on workshop session (the "Workshop") during the Combined Use of Neurotoxins (BoNTA) and Dermal Fillers for Facial Enhancement: Fundamental, Advanced, or Masters Techniques educational (CME/CE) program sponsored by Aesthetic Advancements Institute as part of my participation in the educational program. I will practice facial injection techniques on a volunteer patient of subject (the "Volunteer") prescreened and selected by me, provided by me to be an appropriate candidate for treatment, using US FDA approved injectable agents.
I have knowledge of the risks inherent in the subject facial injections and I hereby assume all responsibility of any kind which may arise in connection with my performance of facial injections, including any or all of my acts and omissions in connection therewith. I hereby agree to indemnify, defend, and hold harmless the program sponsor, Aesthetic Advancements Institute and each of their respective affiliates, licensees, assignees and parent, subsidiary and affiliated companies, and the officers, directors, shareholders, employees and agents of all such entities (all of the foregoing, collectively, the "Indemnities"), from and against any and all costs, losses, liabilities, damages and expenses, including reasonable attorneys’ fees and costs, arising out of any claim, complaint, challenge, proceeding or other action, whether by the Volunteer or otherwise, resulting from or arising out of my participation in the workshop and specifically those medical procedures identified below. I further agree to waive any and all claims of any kind that may arise with respect to Aesthetic Advancements Institute in connection with my participation in the Workshop.
I hold a current license in the state listed in my registration, and with this license I am legally able and plan to perform the following facial aesthetic injection procedures. If training outside my state of licensure, I certify that I fall within the scope of practice for my credentials in the state in which this program is being held for training purposes only. Please note: All cancellations must be received (not postmarked) in writing, via email to admin@aestheticadvancements.com
A refund will be given, less a $250.00 administrative/processing fee if canceled 14 days prior to the start of the course. Any cancellation less than 14 days or "No Shows" will not qualify for a refund. If we are within the 14 day period and we are able to find a "replacement participant", we will gladly switch your course fee to another applicable training date without penalty; however, if a replacement is not found the full amount of training will not be refunded. Due to this policy, weather related cancellations are considered "No Shows" if the program is conducted.
As we do require a minimum number of registrants per each class, and if we do not meet this minimum (7-14 days prior to the training), or for any other reason beyond our control the course is canceled or postponed, the registration fee will be refunded in full. Please note Aesthetic Advancements is not responsible for deposits to hotels, fees associated with necessary airline changes or non-refundable airline tickets.
Classes are filled on a first-come, first-served basis. Some classes fill well in advance, so it is advised to register as soon as possible. We cannot reserve space in this program without receipt of registration packet and payment. You will receive written confirmation from our office when we receive your payment/registration. If you have not received a confirmation within 10 days, please contact us. Your registration is not finalized until you receive a confirmation from our staff. Confirmations are generally sent via the email address you provide on your registration packet. Due to the limited space available in this class, we are unable to accommodate unregistered participants the day of training.
The injectable training sessions include a hands on portion. The products in most courses will not be provided by the organization, therefore, the student is required to bring their own. Products must be legally secured through an FDA approved product manufacturer. Products secured from outside the US will not be permitted. Students who bring products determined to be illegally secured will not be able to participate in the hands-on portion of the class. Refunds will not be provided for any portion of the training. If you have further questions regarding FDA approved products and laws regarding products brought in from outside the US, please contact the FDA directly for additional clarification.
We ask that each registrant check with his/her State Licensing Board and review the practice act pertinent to their scope of practice. We do require you provide your valid state medical or nursing license information with your registration. Please direct any "scope of practice" questions to a member of your state board, as we will be unable to answer questions pertaining to individual state legalities.
Please enter credit card information below to submit registration. Personal checks will not be accepted. Business checks will only be accepted in special instances and must be confirmed through our corporate office at 770-365-3319.
Once we receive your registration and payment, your information will be reviewed by a member of our staff and you will be notified of acceptance within 3 business days. If the course has filled, you will be notified and placed on the waitlist. If you have questions, please contact Anne Marie Camp at 770-365-3319 or acamp@aestheticadvancements.com.
