• Healthy Archuleta Health Partner Survey

  • Overview:  Healthy Archuleta, in partnership with Archuleta County, are working to complete a community health assessment over the next three years. They are looking at two broad questions that will help capture people’s voices/experiences related to food and primary healthcare issues and opportunities in Archuleta County.

    1. How is our community experiencing preventative/primary health care access and utilization (coverage, timeliness, workforce, and service?)

    2. How is our community interacting with our community-based food system and what can be done to bolster our local system?

    As you go through this assessment we are interested in how community members access the preventative/primary care system and how the system interacts to get people what they need.  Primary care providers, mental health providers, dental providers, as well as supporting services in our community will all be asked similar sets of questions to help look at how the system interacts.

    Our Definition of Primary Care: Routine healthcare that includes screenings, check-ups, and patient counseling to prevent illness, disease, and other health problems as well as response to acute illnesses or to coordinate care among specialists
