I understand that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application can result in the forfeiture of any Al's Angel Animal Rescue animal fostered by me.

I certify that the above information is correct, and I understand that the information will be verified.

I understand that by submitting this form electronically, I agree to release and covenant to hold harmless Al's Angels Animal Rescue and it's members from any claims, damages, costs, or actions incurred because of the care or actions of the foster cat or dog.

I accept full responsibility for the cat(s) dog(s) actions at all times, and release Al's Angels Animal Rescue from any liabilities or damages that may be incurred because of fostering such cat(s) or dog(s).

I agree to have Al's Angels Animal Rescue complete reference call checks and conduct a home visit inspection to be able to approve my foster application.

I agree that if I'm unable to foster the cats(s) or dog(s)anymore that I will return the cat(s) dog(s)to Al's Angels Animal Rescue and try to give Al's Angels Animal Rescue a 2-week period to try and find another suitable foster for the cat(s) or dog(s).

I understand that any animal I foster is the property of Al's Angels Animal Rescue.

I understand that Al's Angels Animal Rescue maintains the right to check up on the animal at any time, after giving the foster a 24-hour notice.

I understand that I may not vet the animal for any reason while fostering and agrees to contact Al's Angels Animal Rescue for any medical concerns that I may have.

I understand that tethering outside is not permitted outside any longer than 20 minutes. I understand that I can not leave my foster unattended while outside even in a fenced yard.

I agree not to maintain a safe living environment for my foster at all times. (securing any and all chewing hazards, keeping garbage cans out of reach, etc.)

I agree to monitor my foster whenever around other animals and children.

I agree to not allow other animals or children to tease or taunt my foster. I agree that if my foster would show aggression at any time towards children, I agree to contact Al's Angels Animal Rescue immediately and that my foster will have to be returned to Al's Angels Animal Rescue.

I understand that my foster may not leave the state of WV while in my care.

I understand that if I do not return my foster when Al's Angels Animal Rescue asks for my foster back that I could face legal action and will be responsible for court costs.

I understand that I will provide the food for my foster while he/she is in my care.
