Please complete this application for each position for which your organization is applying for SHARP-1.
Provider Type (select ONLY one)

The Employer Recruitment Prerogative (ERP) is a SHARP provided option for an authorized employer to assert the availability of a support-for-service award, even if the employer is still in its hiring phase. SHARP makes this option available by signing the ERP agreement and the employer is then enabled to offer with certianty the support-for-service benefit. ERP authorization allows the employer to pose the offer of suport-for-service benefit throughout the recruitment process. Go to the web link Employer Recruitment Prerogative for more information.
Do you understand that:
SHARP allows for selected positions to be designated as very hard-to-fill. The default chracterization of a position is regular-fill. However, some employers may face considerable and ongoing difficulty recruiting and retaining practitioners for certain positions. SHARP may designate these positions as very hard-to-fill, which provides additional support-for-service payment to practitioners by increasing the maximum allowable SHARP support-for-service benefit to a level above that provided for a regular position. When an employer requests that a position be designated as very hard-to-fill, the burden of proof that the position is indeed eligible as very hard- to-fill falls to the employer.
Interviews conducted during most recent 12 months, respond once for every non-duplicated interviewee:
SHARP seeks to increase the number and improve the distribution of clinicians who provide substance abuse treatment in Alaska. To do so, SHARP now includes a sub-project for the recruitment and retention of substance abuse treatment clinicians. (For further information see Recruitment and Retention of Substance Abuse Treatment Clinicians)
The following occupations are eligible for the substance abuse initiative. If the position's occupation is not on this list, then it is not eligible for this demonstration. Please select one for which occupation characterizes this position:
SHARP is conducting a demonstration project to recruit longer-term clinicians in order to fill positions now occupied by temporary staffing. The purpose of this demonstration project is three fold: (a) to provide direct assistance to selected healthcare employers, which are struggling with overuse of temporary staffing; (b) to demonstrate the feasibility that longer-term staffing solutions is achievable by the use of support-for-service; and (c) to demonstrate that meaningful cost-savings can be achieved.(For further information about Demonstration-2, see web link Recruitment and Retention of Clinicians to Replace Locum Tenens)
