I attest that my dog has not been to any dog parks, dog daycares, or other dog boarding facilities in the past 21 days.
Should my dog, mentioned above show signs of a contagious disease, such as kennel cough or worms, Owner will be required to pick dog up immediately. If that is not possible, Mile High K-9 has full client approval to treat dog at the expense of client at the Veterinary Clinic of Mile High K-9’s choice, and agrees to dog being medically boarded at the vet's office accordng to their fees, or in isolation boarding at Mile High K-9 at the cost of $100/day.
Should your dog require medical boarding for any reason, Owner is responsible for all costs. Mile High K-9 will not reimburse owner for boarding reservation fees at Mile High K-9 to help cover our time in making sure your pet is properly cared for during this stressful period.
