Pre-Trial Checklist

Trial Form needs to be submitted at least 3 days prior to your child's arrival for the trial class.

Child needs to be dropped off at the school entrance at least 5 minutes before class starts & picked up at the exit after the class ends.

Short videos are taken progressively during class. The videos together with teachers feedback, will be sent to your mobile number to facilitate an informed decision.

We recommend reading our school policies on our website before signing up for the trial class to ensure expectations are aligned.

ASOD Policies

May Open House Promotion:

Register within 3 days of attending the trial class to enjoy a waiver of the trial fee.

Disclaimer: Students are required to attend all 11 lessons in a term. This promo does not apply to prorated term fees.

Trial Class Policies

I understand the placement of my child is based solely on the recommendation of the teacher & is not bound by age group.

There is no refund for a NO SHOW.

Children from early childhood classes are expected to walk in & attend the trial class INDEPENDENTLY without an accompanied parent/adult for an accurate class readiness assessment.

By submitting this form, you, the Parent or Guardian of the above student(s), agree to release "Amy's School of Dance", including instructors and assistants from liability for any and all injuries which may occur while training, practicing, performing, or during any studio event or activity. You also agree that you are responsible for health and accident insurance and any medical costs incurred due to injury. You give permission for emergency medical transportation and treatment of your student(s) at your expense should the need arise. You also give your permission for the public display of any studio visual images that your child may appear in.
