Payment is required at the time of service.

For your convenience, we accept: MasterCard, Visa, Discover, American Express, CareCredit, cash and check (with valid ID).

If you have financial limitations, please let the staff know prior to treatment and, when possible, we will modify your pet's treatment plans to the limits of your budget.

Following examination and a tentative diagnosis, you may be asked to sign a health plan confirming authorization of treatment. The details of treatment, the risks of treatment, and/or the risk of not treating will be explained to you. Please do not hesitate to ask questions about your pet’s veterinary care at any point during examination or treatment. For some treatments or hospitalized care, a deposit may be required. Healthcare plans requiring comprehensive care of more than $500 or more, may require a 25% deposit to begin your pet's treatment.

By signing below you authorize the veterinarians of the Animal Clinic of West Plains to examine, prescribe for or treat the above described pet(s). You assume responsibility for all charges incurred in the care of your pets. You also understand that these charges will be paid at the time of release and that a deposit may be required for surgical treatment. As part of the consideration hereof, You agree to pay all costs of collection, including but not limited to all court costs and reasonable attorney’s fees if this account is placed in the hands of an attorney for collection. You understand that any unpaid balance will be subject to monthly billing and finance charges.
