Lasting Power of Attorney
The following form allows you to provide all the information we require to register your LPAs. The form needs to be completed for each applicant and includes the Health & Welfare LPA and the Property & Financial LPA. The price for this service is £300 payable at the end of the form. Please note: There is a potential subsequent registration fee of £92 per LPA, payable to the Office of Public Guardian directly.
