Due to limitation in hours please do not book an appointment unless you keep your original appointment or otherwise you will be billed for your service. Since most guests book two weeks a head of time, every hour is reserved for a limited amount of guests and canceling within two weeks unfortunately does not give us enough notice to fill that opening. Please do not bring your children, pets or cell phone to appointment. Anytime there is a groupon special we will always offer the same price they are offering, so there is no need to purchase a groupon. Please make sure to arrive to your apt with no eye, face makeup or moisturizer to ensure the lash extensions adhere properly. We recommend not using mascara one day prior to your lash appointment or any type of stick on lash adhesive to ensure quality results. At Sweet Angel we want to ensure we can accommodate you for future visits, so we recommend you book your fill at the studio right after your first service!Thank you!

I understand that this procedure requires single synthetic eyelashes to be glued to my own natural eyelashes. I understand that it is my responsibility to keep my eyes closed and still during the entire procedure, until my eyelash technician addresses me to open my eyes.I understand that some risks of this procedure may be but not limited to eye redness and irritation.The fumes from the adhesive may cause my eyes to tear up if I open my eyes.I agree to disclose any allergies that I may have to latex, surgical tapes, cyanoacrylate, Vaseline, etc. I understand that I am required to follow the eyelash extension care instructions on the sweetangelspa.com website, in order to maintain the life of these extensions.I agree that there are no refunds for the time that was used applying the lash extensions and if I decide I do not want the extensions I may ask for them to be taken off at no charge.I agree that by reading and signing this consent form, I release Sweet Angel Spa Studio from any claims or damages of any nature.
