I am not using any medications that may cause such injury or reaction, and I will promptly inform Anuli Aesthetics if this changes. I Understand That, While Rare, Wellness & Vitamin Injections Can Cause Serious Side Effects. These Reactions Are Uncommon, But It’s Important To Be Aware Of The Possibility. Any Unusual Or Severe Symptoms Should Be Reported To Anuli Aesthetics or a Medical Provider Immediately For Proper Evaluation. Before starting Wellness & Vitamin injections, I agree to inform Anuli Aesthetics immediately if I am pregnant, breastfeeding, or have any of the following conditions: Kidney disease, History of kidney stones, Liver disease, Hypoglycemia, Currently taking blood thinners or medications that affect bone marrow, Any active infection. By signing below, I confirm that I have read and understand the informed consent and agree to proceed with treatment, acknowledging the associated risks. I consent to receive this and any future Wellness Vitamin Injections. I also release Anuli Aesthetics, the medical provider, the administering staff, and the facility from any liability related to this procedure
TERMS: Terms & Return Policy: I certify that I am an authorized user of this credit card and authorize payment for services. I understand that no refunds are offered for medications, as healthcare providers cannot predict individual responses, intolerances, or effectiveness in weight managements. Due to the nature of these medications, medications cannot be returned, cannot be transferred, and non- changeable even if the medication is unused and any monthly fees already paid are non-refundable. Fraud Policy: Anuli Aesthetics has a zero-tolerance policy for fraud, including credit card fraud. Any fraudulent activity will be fully investigated and prosecuted to the fullest extent of the law. Losses incurred due to fraudulent activity will be pursued in civil court to recover all associated costs, including business and legal fees, research costs, and loss of income. Chargeback Policy: A "chargeback" refers to the reversal of a credit/debit card charge. If a credit is due, please contact Anuli Aesthetics & Weight Loss at anuli@anulimedspa.com. Filing a chargeback should never be necessary. By agreeing to our terms, you commit to immediately notifying Anuli Aesthetics if you suspect any fraudulent activity on your credit/debit card related to our services. You also agree to reimburse Anuli Aesthetics for any expenses incurred from a chargeback you initiate. Any unauthorized chargebacks will be pursued by any means necessary, including recharging your card, legal action, or collection agency involvement. Anuli Aesthetics provides medical-grade weight management treatments, including prescription medications, which are non-refundable by law. This policy is clearly outlined in the terms and conditions agreed to at the time of purchase. Medications cannot be returned due to federal regulations and the inability to guarantee their integrity once dispensed. Additionally, the effectiveness, tolerances, and individual responses to these medications can vary and are beyond the control of healthcare providers. As such, any monthly fees or payments made for these treatments are also non-refundable. This policy ensures compliance with legal and medical standards for handling prescription medications. *
