By attending The Magi Project on Nov 18, 2023, I grant permission to The Believe in Three Foundation to capture photographs and videos of me. I understand that these images may be used for promotional purposes, including but not limited to, social media, website contented press releases. I release The Believe in Three Foundation from any liability and waive any rights or claims associated with the use of these photographs and videos.

Acknowledgement, Waiver and Release of Liability

The Believe in Three Foundation Event Waiver of Liability for Minors. As the parent/legal guardian of the named registrant and or registrants of this event, I understand and acknowledge that The Believe in Three Foundation is not responsible for the supervision of minors during The Magi Day of Service at Communion Pointe. Although I may leave the facility while my minor participates, I will be solely responsible for the supervision, safety, and well-being of my minor during the event. I hereby release The Believe Foundation from any liability for any injuries, damages, or losses incurred by my minor during their attendance and participation in the event. I also acknowledge The Believe in Three Foundation and their respective employees, directors, officers, volunteers, members and any other participant entity, party of person involved in any regard with the Activity or the Activity premises and their respective agents, personal representatives, heirs, employees, contractors, successors and assigns (each on “activity representative” and collectively the “activity representatives”), shall not be responsible or liable in any regard or manner for any and all property damage or bodily injury )including serious physical injury or even death) incurred by participant or any party related thereto as a result of his/her participation in the activity. I have reread, fully understand, and hereby freely sign, approve of, and agree to the terms of this registration form. I hereby unconditionally release, discharge, covenant not to sue, waive my rights and remedies, and agree to hold harmless the activity representatives from any and all claims, costs, demands, losses, damages, or expenses associated with, in whole or in part, participant’s involvement with the activity.
