Parent Chaperone or Volunteer Request 23-24

Official copies of all required clearances must be received by the main office before the due date for the student permission slips. We accept volunteers on a first-come, first-served basis OR we may select a parent who has not yet had the opportunity to volunteer if we receive more than we need.

Parents or Guardians are only able to chaperone trips. Extended family are not permitted.

If selected, you will receive an email. Prior to the day of the trip or event, you will be required to sign a Parent Chaperone/Volunteer Agreement. You may preview this form here.

Effective July 2015, all school volunteers must obtain the required clearances. A volunteer is any adult serving in an unpaid position in which they are individually responsible for the welfare of a child or have “direct contact with children.”

For those who wish to be a volunteer, you are required to submit a Pennsylvania Criminal History Record(Act 34), a Child Abuse History Clearance (Act 151) and FBI Federal Criminal History Record to our main office. More information can be found on our website at
